DENTAL VALUE AT EAGLE HARBOR DENTAL
Our high standard dental care for the Dental Value patients is provided by Dr. Fugaro and/or our qualified Register Dental Hygienists.
WHAT IS DENTAL VALUE?
It is a reduced fee-for-service dental program available to individuals and families that do not have dental insurance at Eagle Harbor Dental after the Yearly Visit Fee has been paid.
WHAT DOES THE YEARLY VISIT FEE COVER?
The Yearly Visit Fee includes the first visit necessary x-rays (up to 12), examination, diagnosis, fluoride and teeth cleaning (up to 60% off of our regular fees). In addition, for the one year, you will be entitled to receive a guaranteed 20% discount for the listed services rendered.*

** All family members must be registered at the same time to be able to obtain additional dependent fee. Dependent fee is reserved for individuals up to 23 years old.
HOW CAN I REGISTER?
Complete a Dental Value registration form for yourself and/or your family at the Eagle Harbor Dental office, pay the Yearly Visit Fee(s) the day of registration and you will be a Dental Value member.
HOW CAN I USE DENTAL VALUE?
After the registration form is completed and the Yearly Fee is paid, call Eagle Harbor Dental and identify yourself as a Dental Value patient to schedule an appointment. ***
*** The Dental Value service at Eagle Harbor Dental reserves the right to bill your dental carrier at usual and customary rates for all procedures if you are covered by dental insurance.
WHEN CAN I START USING MY DENTAL VALUE AT EAGLE HARBOR DENTAL?
Treatment may be rendered during the first visit for participants that register and pay the Yearly Visit Fee at the time of the first visit. All family members must be registered at the same time.
COVERED PROCEDURES INCLUDE
Diagnostic, Preventive, Extractions, Oral Surgery, Periodontics, Endodontics (root canal), Restorative Dentistry, Crowns, Prosthetics, Bridges, Dentures (include repairs), Nitrous Oxide administration, Velscope and Nightguards.*

LIMITATIONS
The following limitations apply to participants in Dental Value service at Eagle Harbor Dental:
Any procedures not listed* will be charged at rates no greater than average usual and customary fees at Eagle Harbor Dental.
Treatment rendered outside Eagle Harbor Dental.
Referral to specialists is excluded.
Examination, Cleaning, Fluoride and X-rays must be rendered at the first appointment.
Dental cleaning with the initial membership does not include periodontal treatment.
The fees listed apply only to participants in this preferred provider program. Fees charged to insured patients and others not cover under Dental Value are based on the usual and customary fees charged at Eagle Harbor Dental.
Eagle Harbor Dental reserves the right to adjust Dental Value fees for services rendered annually as determined by published, nationally recognized fee surveys.
Duplication of records fee may be applied to particiants that transfer to another dental provider.
A fee will be applied to cancellations made 48 hrs. prior to your appointment.
Dental Value patients are not elegible to receive our senior and/or cash discount.
